Many of our heart failure physicians and specially trained associates are centrally located at our OhioHealth Advanced Heart and Vascular Center in the McConnell Heart Hospital in Columbus. There’s a lot that goes into keeping a heart healthy. So, we have a lot of specialists who will bring different skills and experience to your care. Your heart failure care team could include:
- Board-certified cardiologists: Our cardiologists are specially trained to diagnose and treat heart failure. They will help you determine if surgical procedures, like a ventricular assist device or heart transplant, are right for you. They can also manage your ongoing care after any heart procedures you have.
- Surgeons: Your cardiologist may recommend a surgical procedure to improve your quality of life, like a coronary artery bypass, valve replacement or ventricular assist device. Our surgeons are experienced in open-heart and minimally invasive procedures. They will work with your care team to ensure you receive the right – and least invasive – treatment.
- Cardiac rehabilitation specialists: Our team of clinical cardiac rehabilitation specialists are experienced in working with patients who have heart failure and other related conditions. They can help you make changes in your life to improve your heart health, like exercising, quitting smoking and managing stress.
- Nurses: Specially trained nurses will monitor your care and check in to see how things are going at home to help you avoid hospital admissions.
- Care coordinators: Nurses who specializes in heart failure care can help you coordinate testing, follow-up appointments, provide education and navigate medical insurance and payments.
- Pharmacists: Our pharmacists will work with your doctor to select the best medication therapies for you and make appropriate adjustments, as needed. They can also answer your questions to help you better understand how each medication works.
- Dietitians: Your diet can affect how hard your heart has to work, so your doctor may recommend nutritional counseling with our dietitians. They can work with you to create a heart-healthy meal plan that fits your lifestyle.
- Health educators: Heart failure, like any health condition, can be complex. Our health educators will help you and your family better understand your condition and recommended treatments.
- Genetic counselors: If you or a family member are worried that heart disease may be hereditary in your family, our genetic counselors can help you determine if genetic testing is right for you.
- OhioHealth at Home: Heart failure can make daily activities, like walking and climbing stairs, more difficult. OhioHealth at Home can bring specialists to you, so you can remain independent longer and potentially avoid hospital admissions.
- Social services: If you need financial assistance, transportation, have housing and food insecurity or could benefit from community resources, our social services specialists can help.
